Pet Age and Weight Requirement: All pets must be at least 8 weeks old and weigh 20 pounds or less to be transported via flight nanny service. Larger pets over 20+ pounds will be delivered via ground transportation. By requesting or arranging transport, the individual certifies the pet meets these requirements as of the scheduled pick-up date. The person signing this form verifies the pet is at least 8 weeks old.

Health Certificate: A valid health certificate, issued by a licensed veterinarian no more than 10 days prior to the scheduled deliveryis recommended for. Vaccinations records are acceptable substitutes and either documents must accompany the pet.

Scheduling – Standby Pick-up and Delivery: An intended flight itinerary will be provided; however, specific pick-up and delivery dates and times are not guaranteed. Unforeseen events, such as flight delays, seating availability, cancellations, weather conditions, or other factors, may necessitate changes to the itinerary.

Scheduling – Confirmed Pick-up and Delivery: A confirmed flight itinerary, including a purchased airline ticket/ pet fee, will be provided. The only factors that may alter this itinerary are flight delays, cancellations, weather, or acts of God.

Pick-up and Delivery Procedure: Reasonable efforts will be made to notify both the sender and receiver of the pet’s estimated arrival time. If the sender or receiver is more than 30 minutes late, a late fee of $50 will apply, with an additional $50 for every subsequent 30 minutes. If the necessary paperwork or responsible party is not present for pickup, there will be no refund issued.
Scheduling and Cancellation Policy: The deposit is non-refundable. Most transports are coordinated well in advance and tightly scheduled. Canceling a scheduled delivery results in forfeiting the total cost for transport. No refund is provided, though funds may be applied to a future transport within 30 days. Exceptions to this policy apply if a flight is canceled by the airline or by QT Pet Transport and rescheduling is not possible within 5 days; in such cases, a partial refund may be granted, not including the deposit.
Rabies Vaccination: A current rabies vaccination certificate is mandatory for all pets being import into the USA from another country.
Food and Feeding Instructions: Please provide a sufficient amount of food in a secure, resealable bag, along with feeding instructions (quantity and frequency). We will provide a travel carrier, pee pads, and water for the pet’s journey. No paper or plastic supermarket bags are allowed.
Allowed Items for New Pet Owners: Only a small, TSA-approved bag of items for the new pet owner is allowed, limited to what can fit in a small backpack. Any items other than food or a small blanket must be pre-approved.
Aggression and Muzzling Policy: We will not transport any pet requiring a muzzle or restrictive device via flight nanny service. The pet must be transported via ground transportation. Our transporters reserve the right to decline transport for any pet displaying aggression.
Cat Transport Requirements: To minimize risk of bites, cats must be in a carrier in a small, contained room or car , allowing easy hand-off. Transporters are instructed not to place cats into carriers; this is the responsibility of the owner or designated contact.
Payment Requirement: The petmust be fully paid for before scheduling. QT Pet Transport does not manage monetary transactions between the seller and buyer.
Direct Contact and Booking Policy: Please do not contact transporters directly for booking inquiries, concerns, or pricing discussions outside of the group text. If transport is scheduled directly with a transporter instead of QT Pet Transport, QT Pet Transport bears no responsibility for that transport and reserves the right to pursue legal action.
Liability Waiver: The booking party acknowledges and agrees that QT Pet Transport holds no liability in cases of injury, harm, or death of the pet, unless the booking party opts into the Company’s in-house insurance.
